PORTFOLIOS:
OVERVIEW:
Each fellow will maintain a portfolio including:
- Pre-fellowship career goals and 5 year plan
- Post fellowship career goals and 5 year plan
- Mid-year learning goals and plan to meet them based on self reflection of deficiencies in medical knowledge of patient care
- Minimum 1 essay that reflects on a difficult ethical dilemma, how the fellow handled the situation, how the fellow felt about the situation, what they learned, and what they would do differently next time
- Minimum 1 patient communication recall of a challenging interaction where fellows will create a script of their conversation with a patient
- Minimum 1 essay of the fellows experience with breaking bad news. This essay should briefly describe the context of the bad news, how it was delivered, and how it was received. Additionally it should examine barriers to communication of bad news and how the situation might be handled differently
- Written research plan and timeline of how to achieve it (to be shared and reviewed with the assigned attending in the first 3 months of the fellowship)
- Written plan for quality assurance project and a timeline of how to achieve it (to be shared and reviewed with the assigned attending in the first 3 months of the fellowship)
- Brief description of your observations during the REACH elective
- 2 brief descriptions of team meetings you attended (one from the hospital and one from the nursing home)
GOALS (competencies):
By the end of the year, fellows will be able to
- Create career goals that challenge them, and foster lifelong learning and a commitment to being a leader in the field of geriatrics (professionalism)
- Prepare them to deal emotionally and rationally with complex ethical dilemmas and challenging communication hurdles (professionalism/patient care/interpersonal skills and communication)
- Design and implement a research protocol (medical knowledge)
- Design and implement a quality assurance investigation (practice based learning)
TEACHING METHODS:
Portfolios will be reviewed by the program director/associate program director. Fellows have the option of sharing either their essays or patient communication recall script at a case conference for teaching and facilitation of communication skills. Fellows are expected to present at least one of these during the year.
METHODS OF EVALUATION:
The concept of portfolios and required elements of the portfolios will be reviewed with the fellows by the program director at the beginning of the year and during the mid year evaluation. Portfolios will be reviewed by the program director/associate program, and fellows will receive formative oral evaluation by the program director/associate program director at the mid-year and end of year review.
